To participate in middle and high school sports, a physical must be completed every year before being allowed to practice with the team. This may seem overkill to some, but this pre-participation physical is the only periodic health exam for 78% of high school athletes.

Usually, the school will give you a form for the doctor to complete. Please have your portion of the Poudre Valley School District forms for your school completed before your visit. If you have misplaced it we have copies available. A Sports Physical History form, will be provided for you to complete in office.

The exam portion of your Fort Collins Sports Physical will cover and exceed the components required by the Colorado High School Activities Association (CHSAA) as well any other sport specific tests deemed necessary depending on your child’s history. We look at your child as an elite athlete in the making and search in depth to uncover potential weaknesses that can cause major troubles later in their athletic career. This is also your chance to discuss with the doctor any other questions you have about your child’s health, prior sports injuries, and other concerns about playing sports and preventing injuries.

Many athletes, once they have been cleared to participate with their sports physical, choose to fine tune their bodies to prevent injuries and optimize performance by going through a functional exam to determine the areas that are holding your child back from being the best they can.
